I am the parent or legal guardian of the player identified above and authorize the player to participate in Back End Hockey training activities.

MEDICAL INFORMATION

I agree to disclose relevant allergies, medical conditions, injuries, medications, restrictions or other information that may reasonably affect the player’s safe participation. I understand that Back End Hockey is not a medical provider and that coaches may rely on the information supplied by me.

EMERGENCY CARE

If I cannot be reached in an emergency, I authorize Back End Hockey personnel to contact emergency medical services and to take reasonable steps to obtain urgent medical care for the player. I understand that I remain responsible for any medical, ambulance or related costs not otherwise covered.

PARTICIPATION

I will not knowingly send the player onto the ice when the player has been medically advised not to participate or when participation would be unsafe due to illness or injury.

In consideration of the player being permitted to participate in hockey training, skating, drills, instruction and related activities provided by Back End Hockey (the “Program”), the undersigned parent or legal guardian acknowledges and agrees as follows:

ASSUMPTION OF RISK

I understand that hockey and on-ice training involve inherent risks, including falls, collisions, contact with other players, sticks, pucks, boards, ice surfaces and equipment. Injuries may include bruises, cuts, sprains, fractures, concussions, serious injury, permanent disability or, in rare circumstances, death. I voluntarily accept these risks on behalf of the player.

PLAYER RESPONSIBILITY

I confirm that the player will wear appropriate hockey equipment and will follow reasonable safety instructions given by coaches, instructors and facility staff. I will disclose any condition or restriction that may affect safe participation.

RELEASE AND INDEMNITY

To the fullest extent permitted by applicable law, I release and hold harmless Back End Hockey, its owners, coaches, instructors, volunteers and representatives from claims arising from the ordinary risks of participation in the Program, except to the extent a claim cannot legally be waived or released.

I agree to indemnify and hold harmless the released parties from claims resulting from inaccurate or incomplete information provided by me, or from the player’s intentional failure to follow reasonable safety instructions, to the extent permitted by law.

ACKNOWLEDGEMENT

I confirm that I am the player’s parent or legal guardian, that I have read and understood this document, and that I am agreeing voluntarily.

CONCUSSION AWARENESS

A concussion is a brain injury that may be caused by a blow to the head, face, neck or body. Symptoms may appear immediately or later. A suspected concussion must be taken seriously.

Before registration, the athlete and, when the athlete is under 18, the parent/legal guardian should review the applicable Government of Ontario Concussion Awareness Resource. The appropriate resource is based on the athlete’s age.

CODE OF CONDUCT

By participating with Back End Hockey, athletes and parents/legal guardians commit to fair play and respect; helping prevent concussion by following hockey rules and safety instructions; recognizing and reporting possible concussion symptoms; not hiding or minimizing symptoms; respecting any decision to remove an athlete because a concussion is suspected; and following Back End Hockey’s removal-from-sport and return-to-sport requirements and any medical direction before returning to unrestricted participation.

REMOVAL FROM SPORT

If Back End Hockey becomes aware that an athlete has sustained, or is suspected of having sustained, a concussion, the athlete will be removed from further training, practice or competition in accordance with the organization’s concussion protocol. For a minor athlete, the parent/legal guardian will be informed. The athlete must not return simply because symptoms improve during the session.

MEDICAL ASSESSMENT + RETURN TO SPORT

An athlete removed because of a suspected concussion must follow Back End Hockey’s return-to-sport protocol. Where medical assessment or clearance is required under applicable Ontario concussion requirements, assessment and medical clearance must be provided by an authorized physician or nurse practitioner. Return to sport must be gradual and must not occur until the applicable requirements have been satisfied.
